Dr. Patrick M. Cusack is a Peoria native. He graduated from Bergan High School in 1986 and then did his undergraduate work in mechanical engineering at Iowa State University. He graduated from the University of Louisville School of Dentistry in 1995, earning his Doctor of Dental Medicine degree (DMD). Dr. Cusack earned the Master of Science degree in Orthodontics at the St. Louis University Graduate Orthodontics Program.

Active in organized dentistry, Dr. Cusack is a member of the following organizations:

• The Midwestern Society of Orthodontists
• The American Dental Association
• The Illinois State Dental Society
• The Peoria District Dental Society
• The Chicago Dental Society

Away from the practice, Dr. Cusack enjoys golf, boating, computers, and spending time with his wife, Michelle, and their three daughters, Caitlin, Courtney, and Megan.
